Aunt Vadge: my vagina itches and burns after antibiotic treatment for a sinus infection

Hi Aunt Vadge,

I was on antibiotics for a sinus problem and now I’m uncomfortable – an itchy, burning sensation, with some cuts and tears, mostly on the outside (no discomfort inside). I also have a dark brown discharge, but I get that every other month and it lasts till my next period.

I’m on birth control and have been for nine years. Can you tell me why I get the discharge so often? It’s making me avoid my partner when he wants to be romantic. Any advice on that and the itchy, burning feeling?

Two things are going on, and they’re separate. The itchy, burning, slightly-cut soreness on the outside, coming right after a course of antibiotics, is the classic signature of a post-antibiotic yeast flare: antibiotics knock down the protective bacteria that keep yeast in check, so it blooms and irritates the vulval skin, sometimes splitting it into little fissures, often without much discharge.

So that part is very likely yeast – common, harmless and very treatable. Our yeast page has gentle home options, and I’d go easy on repeated pharmacy antifungals where you can; strip back to plain-water washing and fragrance-free everything while it settles, and don’t scratch.

The dark brown discharge is a different question, and because it happens every other month and lasts till your next period, I’d get it checked rather than just reassured.

Brown discharge is old blood, and on the pill – especially after nine years – recurrent breakthrough bleeding between periods is common and often just the lining being a bit unstable on long-term hormones.

But recurrent, unexplained bleeding deserves a doctor’s look at your cervix and a check that everything’s as it should be, partly because it’s the sensible thing at this frequency, and partly because it’s the thing making you avoid your partner, which you don’t have to put up with.

So treat the yeast for the itch and cuts, and book a routine check for the brown discharge and your contraception – there’s often an easy fix (sometimes just switching pill) once someone’s had a look.
