Hi there Aunt Vadge,

I’ve had issues with sex for three years. Penetrative sex has never worked – it’s as if I’m so tiny nothing can enter, and my boyfriend isn’t particularly large. We’ve tried for so long it’s caused a lot of frustration and even painless labia minora tears that have changed the landscape of my vagina. We’ve researched endlessly and I’m aware it might be vaginismus, but I can take up to two fingers, and I don’t even know if sex hurts because it’s never been achieved.

Three years of this is exhausting and demoralising, so let me start clearly: this is common, it has a name, and it’s treatable. You have not “failed,” and the belief that it’ll never work isn’t the truth – it’s the frustration talking.

What you describe fits vaginismus closely: the pelvic-floor muscles around the entrance tighten involuntarily, so penetration meets a wall even though you’re not consciously clenching, and even though you can manage two fingers at other times. That mismatch – two fingers fine, but penetrative sex impossible – is very typical, because arousal, position, anticipation and anxiety all feed the guarding.

The recurring labia minora tears point to a friction-and-fragility layer on top, too – likely not enough lubrication and a lot of forcing against tight, tense tissue over the years. So part of the plan is simply to stop the repeated trying-and-tearing, which keeps the area sore and keeps your body braced for pain.

The single most effective step is a pelvic-floor physiotherapist – this is precisely what they treat, using graded dilators, relaxation and desensitisation work, often alongside a psychosexual therapist for the anxiety side. That combination resolves vaginismus for the large majority of people, and it works far better than the two of you continuing to research and push alone.

So please take penetration off the to-do list for now (that pressure is part of what tightens everything), get a referral to a pelvic-floor physio, use plenty of lubricant for any kind of touch, and let the tears fully heal. You’re not too small, you’re not broken, and “it’ll never work” simply isn’t where this ends with the right help.
